Chronic Disease Management

At St. Paul’s Hospital, we use a holistic model of care to support patients as they navigate their individual care needs while living with chronic conditions. Our Chronic Disease Management program assists patients and families through education, holistic health programs and connections to resources across the community.

You have the strength to live well with a chronic condition — and we’re here to support you every step of the way. Our interdisciplinary team partners with you, offering:

  • Supervised exercise programs to support strength, energy and safe activity.
  • Counselling and emotional support for you and your family.
  • Practical tools and strategies to build resilience, manage symptoms and stay engaged in daily life.

We believe in empowering you with knowledge and support, so you can take an active role in your care while knowing help is always here when you need it.

We work with individuals and families living with chronic conditions across the Saskatoon area, including those with kidney disease, diabetes and other long-term illnesses.

Our services include group education sessions, one-on-one counselling, disease-specific clinics, supervised exercise programs and self-management workshops.

We collaborate with healthcare providers and community programs to ensure coordinated care and support. Our focus is on empowering patients to build confidence and independence in managing their health.

The Chronic Disease Management program is located in B Wing, 3rd Floor at St. Paul’s Hospital.
